Biosimilar Drugs For Cancer Treatment

Immunotherapy and targeted therapy drugs are examples of biologic drugs used in cancer treatment.

Generic drugs work the same way and can be used in the same ways as their brand name drugs.

While all biosimilar drugs go through the same initial FDA approval process, and they can be used to treat a disease once they get initial approval, it needs another, special FDA approval to be considered interchangeable before it can be substituted automatically for its brand name drug.

A biosimilar drug that is not approved as interchangeable needs a prescription to be written specifically for the biosimilar to be used instead of its brand name drug.

Researchers and Congress think one benefit to biosimilar drugs is that they might lead to lower drug costs by offering patients more options for treatment.

It also depends on what types of diseases are able to be treated with biosimilar drugs and how much the approved drugs are used.

In the next several years, the number of biosimilar drugs approved to treat cancer is expected to go up.

What Is A Biosimilar?

A biosimilar is a manufactured biologic considered similar enough to an existing medicine that regulators can’t find a clinically meaningful difference from the reference product.

At least that’s been the trend outside the U.S., where regulatory pathways to biosimilar approval have been in place much longer.

The FDA granted approval to the first U.S. biosimilar, Zarxio from Novartis, on March 6, 2015, just a few weeks ahead of the ACA’s fifth anniversery.

Because of a mandatory six-month waiting period following biosimilar approval, Zarxio, which references Amgen’s Neupogen, didn’t begin its commercial launch until September 2015 and hasn’t announced its sales figures in the U.S. yet.

In the EU, the same biosimilar earned marketing approval under the name Zarzio in 2009, and Amgen’s annual sales of Neupogen recorded outside the U.S. have slid from a high of $445 million in 2008 to $256 million in 2015.

Based on the gradual erosion of sales we’ve seen outside the U.S., biosimilar sales are going to be more of a thorn in biologic drugmakers’ sides than bullet wounds.

Biosimilars have found it difficult to get market share, which led biosimilar developer Pfizer to sue Johnson & Johnson over anticompetitive contracts with pharmacy benefit managers which bundle discounts; these are sometimes called the “Rebate wall”, and the rebates are generally unavailable to customers.

Even though the biosimilars market is rising, the price drop for biological drugs at risk of patent expiration will not be as great as for other generic drugs; in fact it has been estimated that the price for biosimilar products will be 65%-85% of their originators.

The specific indication that was withdrawn was for the use of bevacizumab in metastatic breast cancer, with paclitaxel for the treatment of people who have not received chemotherapy for metastatic HER2-negative breast cancer.

The progression-free survival intervals were similar in the bevacizumab-erlotinib and bevacizumab-chemotherapy arms [].In MBC, a single-arm, phase II study of bevacizumab in combination with docetaxel and trastuzumab showed a PFS interval of 7.5 months []. The Commissioner’s decision related to breast cancer will not affect Avastin’s approved indications for use in colon, lung, kidney, and brain cancers.

We are at the beginning stages of this process and the agency will review the materials in order to make a determination on whether a public hearing for Avastin in metastatic breast cancer is warranted.

Avastin for use in women with metastatic breast cancer is still an FDA-approved indication.

Avastin is an injectable cancer medication that works by blocking a protein that is important for the formation of blood vessels.

Avastin was first approved in 2004 for treatment of advanced colon cancer and has been approved since for advanced lung, kidney and brain cancers.

The announcement will not have any immediate effect on the approval of Avastin to treat metastatic breast cancer and there will be no change to product labeling.

Bevacizumab

Diabetic Macular Edema – The Pan-American Collaborative Retina Study Group trial compared monthly intravitreal bevacizumab with macular focal-grid laser photocoagulation and showed an anverage of 11.86 letters gained with bevacizumab and 3.66 letters gained with focal grid laser over 24-months.

It is unknown whether intravitreal bevacizumab is associated with the same black box safety issues however these types of complications are not included in the known complications following intravitreal used of bevacizumab.

Much larger retrospective reviews show the rate of infection to be between 0.022% and 0.16%. One such large review, looked at over 26,000 injections of both bevacizumab and ranibizumab and showed an over all infection rate of 0.02%. Interestingly, both the bevacizumab and ranibizumab groups had 2 cases of endophthalmitis for a 0.02% infection rate for each group.

The IVAN trial showed serious adverse effects to be 12.5% in the bevacizumab group while the CATT trial showed SAE rate of 40%. The Bevacizumab or Laser Therapy trial, which compared bevacizumab versus macular laser, showed approximately 5% SAE in the bevacizumab group and 10% in the laser group.

Ranibizumab has a higher affinity to binding VEGF than does bevacizumab, however bevacizumab has two binding sites per molecule, while ranibizumab only has one.

It has been shown that at clinical doses, bevacizumab and ranibizumab are equally potent at binding VEGF. The incidence of serious systemic adverse effects are rare with both intravitreal bevacizumab and ranibizumab.

The CATT and IVAN trials showed no difference in treatment effect between these two drugs in AMD. However, due to the longer half-life, bevacizumab also persists longer systemically in the serum, which may be an explanation for the increased systemic adverse effects seen in the CATT trial in the bevacizumab groups, if indeed this was a real effect.

What is CPVC? Conceptually, CPVC is a PVC homopolymer that has been subjected to a chlorination reaction.

Delta Lifeboat captain John Horton wants to continue offering the vessel’s lifesaving services – but needs the public’s help to ensure the crew can continue.

In addition to saving lives, the Delta Lifeboat, which is part of the Canadian Lifeboat Institute, also offers volunteer support and assistance to Delta’s emergency services, the Canadian Coast Guard, Royal Canadian Marine Search and Rescue, fishermen, commercial shipping and recreational boaters.

In addition to limited fundraising abilities, unexpected maintenance costs for CLI’s two vessels, the Delta Lifeboat and Fraser Lifeboat, has pushed them to put out an “Urgent appeal” to raise $40,000 for the Delta Lifeboat’s expenses.

“We have to make sure that our lifeboats are, of course, in tip top condition, so that we can continue to do our lifesaving work. The Fraser River here, which is where we operate two of our lifeboats out of the mouth of the Fraser River, it is actually one of the leading search and rescue areas in the whole of Canada. There’s a lot of lives lost here over the years, so it’s really important that we support the Coast Guard and other search and rescue organizations,” says Horton.

The funds will go towards fuel, training crew members and all things needed to make the Ladner Harbor-based Delta Lifeboat functional in its many roles.

Since the pandemic, Horton has noticed a significant increase of people buying recreational vessels, particularly to go fishing off the mouth of the Fraser River.

“A lot of these people are absolutely brand new to boating … there’s a lot of people out there that don’t know what they’re doing, which has added not only to us helping people, but having to be there and keeping our eyes open to make sure that if somebody gets into trouble, there’s assistance at hand,” he says.
