With an increase of patients experiencing respiratory symptoms in light of the current pandemic, it is particularly important to be well-informed about this treatment and how to safely provide it.

Noninvasive Ventilation: Overview, Methods Of Delivery, General Considerations

In its simplest terms, noninvasive ventilation differs from invasive ventilation by the interface between the patient and the ventilator.

While orofacial masks and nasal masks are the most commonly used interfaces, other patient ventilator interfaces through which noninvasive ventilation can be applied include mouthpieces, nasal pillows, total face masks, and even a helmet device, which encompasses the entire head. Experience with helmet devices is limited but increasing, and it has been successful in patients who are unable to tolerate the nasal or orofacial devices.

Most patients who are provided noninvasive ventilation are provided support with pressure ventilation, with continuous positive airway pressure, which is the most basic level of support.

The following provides some guidance on titration of ventilator settings in patients with respiratory distress and who have never been placed on noninvasive ventilation.

Importantly, recognize that certain parameters may predict successful noninvasive ventilation or failure of noninvasive ventilation, so that patients are not subjected to continued treatment when optimal treatment requires intubation and mechanical ventilation.

The changes, in turn, are a reflection of the patient’s ability to cooperate with noninvasive ventilation, patient-ventilatory synchrony, and noninvasive ventilation effectiveness.

Trials of noninvasive ventilation are usually 1-2 hours in length and are useful to determine if a patient can be treated with noninvasive ventilation.

Duration Of Noninvasive Ventilation Is Not A Predictor Of Clinical Outcomes In Patients With Acute Exacerbation Of COPD And Respiratory Failure

Noninvasive ventilation is proven to be effective in the majority of patients with acute exacerbation COPD complicated with respiratory failure.

In particular, we wondered whether duration of NIV among those who eventually failed was associated with poor outcomes.

Additional chart review was performed to collect NIV setting parameters on presentation to the MICU. Clinical outcome variables collected included outcome and duration of NIV, duration of invasive mechanical ventilation, MICU length of stay, hospital length of stay, and in-hospital mortality.

Patients had mean pH of 7.20 0.54 and PaCO2 of 70.3 28.7 on presentation; 323 patients were successfully weaned off NIV; 47 patients failed NIV and required invasive mechanical ventilation.

APACHE III score was higher among patients who failed NIV. In the subset of 47 patients who failed NIV requiring intubation, duration of NIV was 25.0 58.8 h. Multivariate regression analysis yielded a model consisting of APACHE III score and body mass index as predictive variables for NIV failure.

Duration of NIV was not associated with worse clinical outcomes among patients who failed NIV. Conclusions: NIV is successful in preventing invasive mechanical ventilation in majority of patients with acute respiratory failure due to COPD. Patients with worse clinical status at presentation are more likely to fail NIV and require mechanical ventilation.

In the subgroup of patients who failed NIV, duration of NIV prior to intubation was not associated with poor clinical outcomes.

The move comes after Didi failed to appease Chinese regulators in an ongoing cybersecurity probe, as the country demands the company overhaul systems used to house sensitive user data, Bloomberg reported, citing people familiar with the matter.

The Friday downturn in Didi extended an ongoing decline in Chinese stocks, which were under pressure on Thursday after the SEC identified five Chinese companies that could be delisted if they don’t allow US-based accountants to audit their financials.

Didi went public on the New York Stock Exchange last summer in a move, angering Chinese regulators who had given last-minute warnings not to move ahead with the planned IPO. The company has since announced plans to delist its stock from the New York Stock Exchange and relist in Hong Kong.

Chinese electric vehicle maker Nio Inc. has received approval from the Hong Kong Stock Exchange for a secondary listing of its Class A ordinary shares.
