Basosquamous or metatypical BCC shows features of both BCC and SCC. The exact nature of this lesion is controversial, but most reserve this term for the rare basal cell carcinoma composed of nests and strands of cells maturing into larger and paler cells without peripheral palisading.

Many clinical variants of BCC exist, but the most recognized types are superficial, nodular, and morphea-like BCC. Nodular BCC is the most common.

Portions of superficial basal cell carcinomas can evolve into nodular BCC over time.

A skin biopsy is necessary for clinical confirmation of BCC. A shave, punch, or excisional biopsy are all options, taking care to include some portion of the dermis in the specimen to differentiate between superficial and other invasive histologic subtypes of BCC. It should be noted that punch and shave biopsy techniques are about 80% accurate in diagnosing the various subtypes of basal cell carcinoma.

The recurrence rates for primary BCC are as follows: Mohs surgery, 1.0%; surgical excision, 10.1%; EDC, 7.7%; radiation therapy, 8.7%; and cryosurgery, 7.5%. Mohs surgery provides the best long-term cure rate of any treatment modality for BCC. MMS is the gold standard for treating high-risk BCCs and recurrent BCCs because of its high cure rate and tissue-sparing benefit.

Bausch Health Draws ‘Untitled Letter’ From OPDP For DTC Video, HCP Web Page

Bausch Health has drawn scrutiny for a promotional video and web page that, regulators said, misbranded its prescription psoriasis cream Duobrii.

Bausch was targeted because the video and web page downplay the risks associated with the drug and mislead regarding the overall benefit patients may expect, according to an untitled letter sent by the FDA’s Office of Prescription Drug Promotion.

Untitled letters, a less severe form of enforcement than warning letters, notify the manufacturer of a regulatory violation.

The letter to Bausch Health is the second of its kind in 2022, following four in 2021.

Like the January 2022 untitled letter for social media promotion around Eli Lilly’s Trulicity diabetes injection, the Bausch letter comments not simply on the omission of risk information but the manner in which that information is being presented.

While some information regarding the risk is presented in the video, it “Does not mitigate the misleading impression,” the letter reads.

Bausch Health is required to submit a response to OPDP within 15 working days.

Methods: We searched Medline and Embase through March 2020 for randomized trials with a minimum 2-week intervention period that compared MCT oil with another fat or oil.

Outcomes were total cholesterol, LDL cholesterol, HDL cholesterol, and triglyceride levels.

MCT oil intake did not affect total cholesterol, LDL cholesterol, or HDL cholesterol levels, but did increase triglycerides.

Subgroup analyses showed that the effects of MCT oil on total cholesterol and LDL cholesterol differed based on the fatty acid profile of the control oil, with MCT oil increasing total cholesterol and LDL cholesterol when compared to a comparator consisting predominantly of unsaturated fatty acids, and with some evidence for reductions when compared to longer-chain SFAs.

Conclusions: MCT oil does not affect total cholesterol, LDL cholesterol, or HDL cholesterol levels, but does cause a small increase in triglycerides.

The company’s revenue prospects could dim further if the Securities and Exchange Commission banned the controversial practice of payment for order flow, David Trainer, the CEO of investment research firm New Constructs, told Forbes.

High-profile proponents of regulating the practice abound, including best-selling author Michael Lewis.

In a new interview, Lewis said the SEC should make a “Big change” on payment for order flow.

Lewis, whose landmark 2014 book “Flash Boys” drew attention to the lucrative use of high frequency trading on Wall Street, said that brokers who use payment for order flow are “Skimming rent” and “Getting rich.”

When accepting payment for order flow, brokers accept smaller price improvement for trades in exchange for higher payments from the market makers that fulfill the trades – a practice that can amount to a small surcharge for traders, effectively nullifying the commission-free trading promised by some brokers.

“It’s really true that it’s cheaper to trade in the stock market now than it was a long time ago, when you paid a broker a ridiculous commission to execute your trade,” Lewis says.

To be sure, Robinhood is hardly the only broker that accepts payment for order flow.
